Letters & request templates

Getting services for your child often comes down to putting the right request in writing. These templates give you a professional, accurate starting point for the most common letters Alabama parents need — for schools and for insurance. Pick one, fill in your details, and copy, download, or print it.

Everything you type stays on your device

This tool runs entirely in your browser. Your child's name, date of birth, school, insurance details, and anything else you enter are used to build the letter right here on your screen — they are never sent to us, saved, or stored. When you close or refresh this page, the information is gone.

About this letter: Request a special-education evaluation

Under the federal IDEA law, you can request in writing that your public school evaluate your child for special education at no cost to you. Your written request is important — in Alabama, the clock starts when you give written consent, and the evaluation must be completed within 60 calendar days of that consent. The school must evaluate in all areas of suspected disability. Send this to your school's special-education coordinator or principal, and keep a dated copy.

2. Fill in the details

Leave anything blank and the letter will show a clear bracketed note (like[your child's name]) so you can fill it in by hand later.

November 5, 2026 [name or title of recipient] [school name] [school district name] Re: Written request for a special-education evaluation — [your child's name] (DOB [your child's date of birth]), [grade] Dear [name or title of recipient], I am the parent/guardian of [your child's name], who attends [school name]. I am writing to formally request, in writing, that the school conduct a full and individual initial evaluation of my child to determine eligibility for special-education and related services under the Individuals with Disabilities Education Act (IDEA). I have reason to suspect that my child has a disability that is affecting learning and access to education, and I am requesting that the evaluation assess my child in all areas of suspected disability. I understand that under IDEA, and under Alabama's special-education procedures, once I provide written consent for the evaluation the school has 60 calendar days to complete it. Please send me the consent-for-evaluation form and the procedural-safeguards notice so we can begin promptly. Please also confirm in writing that you have received this request and the date you received it. I am happy to meet, share information, and participate in the process. You can reach me at [your phone number and email]. Thank you for your prompt attention to my child's needs. Sincerely, [your name] [your phone number and email]
